Modem Activation Form

This article walks through how to access and fill out the Modem Activation Form

To access the form, scan the QR code on top label in the upper right corner of the inside of the modem door.

Note: The QR code is unique for every modem. Do not reuse a QR code link from a different modem, this will overwrite the information provided for that modem.

Field 1: Serial Number

When you have opened the form, please ensure that the first field matches the serial number on the label on the modem.

Because this field is unique to the modem, it cannot be changed. If this field do not match what is printed on the label, please contact Customer Success.

Field 2-4: Company

If you are not registering the modem with the company listed in field 2, please specific that in field 3 and enter the company that is taking ownership of the modem in the field 4.

Field 5 & 6: Name and Location

Provide the name and location you would like the modem to be listed as in RMS.

Field 7-10: Controller Information

Field 7-9 is for your controller's name and serial number. You can also set how many controllers will be connected to the modem; this will provide additional fields for the name and serials numbers.

Field 10 is for controller connection configuration. If you plan to connect over Modbus, BACnet or another HMI protocol, please select Yes for this field.

Field 11: Time Zone

Use this field to set the time zone in which this modem is located.

Field 12-15: Contact Information

These fields are for contact information, for if we need to reach out to you regarding the modem. We ask that you provide you first and last name, email, and optionally, your phone number.
